This will also include one 20 minute virtual call to check in and answer questions and get email updates on progress at 1 month and 2 month post initial visit. Email questions will be answered during business hours.

Please download the history form(s) pertaining to your reason(s) for treatment. Please bring all completed forms with you to your initial office visit. We also encourage you to bring any medical history, reports, and lab evaluations with you to your first visit.
We understand that unexpected situations may arise, and adjusting your schedule can sometimes be necessary. However, to manage your appointment effectively and to respect the time of our service providers, we kindly ask you to adhere to the following cancellation policy:
All clients must give at least a 24-hour notice for cancellations. If you need to cancel or reschedule your appointment, please do so at least 24 hours prior to your appointment time.
Appointments cancelled with less than 24 hours’ notice will be charged in full. This includes no-shows and late cancellations.
If you have an emergency, we understand that the 24-hour policy may not be feasible. In such a case, please contact us as soon as possible to reschedule your appointment. The decision to waive the cancellation fee will be considered on a case-by-case basis.
Please remember, when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time. We appreciate your understanding and adherence to this policy.
